2010 Community Connections Collaborators

In our day-to-day program delivery, Community Connections collaborates with a wide variety of service providers and non-profit agencies to maximize program delivery and prevent duplication of services. On-going collaborators include:

The City of Jacksonville

  • Jacksonville Children’s Commission – provides staff training, funding for childcare
  • City of Jacksonville Mental Health & Welfare Division – provides financial assistance to residents for past-due rent
  • City of Jacksonville Dental Health Van – visits annually
  • Duval County Schools System – Project Access provides no-cost tutoring services, also a social worker for the homeless children; also bus tokens for children
  • Early Learning Coalition of Duval County – Voluntary Pre-Kindergarten funding
  • Duval County Health Dept. Magnolia Project – STD screening and family planning for women of childbearing ages
  • City of Jacksonville Neighborhoods Dept. – provides grant funding (have funding from theCDBG block grants for the past three years)
  • Public Housing Authority – provides Section 8 housing on a priority basis

U.S. Department of Housing & Urban Development– HUD sponsors, in part, Community Connections housing program

United Way – provides referrals, NEFIN system, and program funding

I.M. Sulzbacher Center for the Homeless – The Davis Center provides childcare for Sulzbacher residents; in return, the Sulzbacher Center provides meals to some Davis Center residents, as well as healthcare, dental care, and a pediatric health clinic

Trinity Rescue Mission – we take their client referrals and children in our childcare

Family Foundations  – provides Credit When Credit is Due, a 6-week finance classes

Family Nurturing Center– provides parenting classes for families at risk of child abuse

Hubbard House – provides domestic violence classes and client referrals

Jewish Family and Community Services – provides deposits, rent and utility assistance

Jacksonville Jaguars Foundation – The Jacksonville Jaguar Foundation is committed to serving the greater Jacksonville area through strategic financial, networking and volunteer support to benefit economically and socially disadvantaged youth and families

Catholic Charities – provides financial assistance to residents in need

River Region Human Services – provides staff and resident training regarding substance abuse; after-care is provided by RRHS staff for residents who have completed treatment

Gateway Community Services – provides after-care for women who have completed treatment and client referrals

Dignity U Wear – provides new clothing quarterly

St. Vincent’s Mobile Health Van – visits bi-monthly, assists clients with no insurance

Goodwill of North Florida –assist residents in finding work, and offers 4-day workshops on improving employability

Local colleges – (UNF, FCCJ, JU) provide student volunteers and interns; UNF provides nursing interns; FCCJ provides contract literacy training

The Women’s Center – provides classes and individual counseling

Volunteers In Medicine – provides health care for qualified working residents

MOCA-Jax – provides on-going interactive arts project for children and their parents

KidsNCare – healthcare for homeless children, taxi vouchers for transport

Nemours BrightStart! – dyslexia screening and preliteracy training for preschoolers

Shands – healthcare for qualified residents after 90 days

City Rescue Mission – vouchers for up to one room of furniture, client referrals, and the Davis Center offers childcare to their residents’ children

Experience Works – provides seniors as volunteers; utilized as receptionists

Salvation Army –  client referrals

Liberty Center III – referrals to their program for permanent housing

Urban League – provides employability skills and job retention workshops, on-site weekly

WorkSource – job training and food stamps

Volunteers of America – takes referrals for women diagnosed with mental illnesses

Planned Parenthood – provides classes using its “Healthy Choices” curriculum

The Kennedy Center – provides vouchers for work apparel, life coaching, and rental assistance

Second Harvest Food Bank – provides subsidized food provisions, free food to women who qualify, and snacks

Publix Food Stores – provides food, bread, pastries, etc. weekly

Kentucky Fried Chicken – provides food weekly

Winn-Dixie – Summer Camp scholarships for homeless and low-income children

Stockton Eller Design – web site design and services
